Consent for the Assessment to Proceed

The Herts Ability driving assessment process has been explained to me and I agree for the assessment to proceed.

This included:

Legal eyesight standards
Theory Test screening
Driving
Physical adaptations if required

The possible outcomes from this assessment may include:

Safe to start or continue learning to drive
Progress review after starting tuition
Seek Theory Test support
Recommendation to pause the learning to drive process
Recommendation to seek clinical advice before continuing
